New Home Warranty Service Requests must be submitted in writing. Please use this form for all your warranty service requests. Requests made in writing by mail or fax will also be accepted. Verbal requests made by telephone will not be accepted, except in the case of extreme emergencies. General maintenance matters, which are the responsibility of the homeowner, will not be considered as New Home Warranty service items.

Directions for form use: If you have Outlook or Outlook Express when submitting the form hit the "send by email" button. If you use a webmail program (i.e., Yahoo, Hotmail), click on "save" and save the form to your computer. Go to your email program and attach the document and send to maintenance@elliotthomes.com.
